Farmingville Trick or Treat Sign-Up

Complete the form below to pre-register for the Trick or Treat Trail. All Trick or Treaters must be pre-registered. If you are not trick or treating for candy, you do not need to register. The entry fee is $15 per trick or treater. Parents are welcome to escort their children without paying. Complete the form, hit SUBMIT, and you will be directed to paypal for payment. Payment can be made via credit card, debit card or paypal. You must register each child separately via the form below.

There is no rain date this year. If the weather does not cooperate, you should arrive at your designated time on October 28th, head to the 1850 Historic Schoolhouse, and each registered child will receive the clues and a bag of our historic candy. If the rain plan has to be implemented, we will notify everyone via email and it will be published on the website and social media.

Please read the following statement, and check the box to approve and proceed: In consideration of your accepting this entry, I, intending to be legally bound, hereby waive and release and hold harmless Farmingville Historical Society, Inc., Suffolk County, all sponsors and their representatives, successors and assigns, for any and all liabilities, claims, demands and causes of action whether or not arising in whole or in part out of the negligence of any of the above organizations or individuals. If approved by a parent or guardian, the parent or guardian agrees to release and hold the above-named organizations and personnel harmless of any claims and rights which may be asserted on behalf of the entrant. Further, I hereby grant permission to any and all of the foregoing to use any photographs, video, recordings or any other record of this event for any purpose whatsoever.
